Upon discovery, the CCHCS Privacy Office took immediate steps to investigate and mitigate the breach. These actions included instructing the recipient to permanently delete the email and any related files, and obtaining confirmation that the information was not accessed or further disclosed. CCHCS also reported the incident to the appropriate regulatory authorities and required the involved employee to complete additional privacy and information security training. The compromised PHI included individuals’ last names, CDCR numbers, risk/priority levels, order types and names, reasons for appointments, and appointment dates. CCHCS has since begun notifying affected individuals in accordance with applicable privacy laws and regulations.
