To Whom It May Concern:

I am a driver for Uber. I am writing to request Uber provide to my attorney a copy of my personnel records, including but not limited to:

Records related to my interaction with and use of Uber’s Driver app, including my trip histories, app usage records, payment statements, and records of my earnings;
When a piece rate or incentive plan was in operation, records of piece rates and an explanation of the incentive formula;
Any Platform Access Agreements or other similar agreements accepted by me, and records reflecting the process by which I signed those agreements;
My itemized wage statements;
My meal and rest break records;
A copy of all other agreements entered into by me and Uber Technologies Inc.;
Any other documents relating to the obtaining and holding of my employment with Uber Technologies Inc.

This request is made pursuant to Labor Code Sections 226, 432 and 1198.5 and IWC Wage Order 9, Section 6(C) Please provide this information to my attorneys at the below address within 21 days of your receipt of this letter:

In addition, under the California Consumer Privacy Act (CCPA), I request Uber Technologies Inc. disclose all personal information that it has collected, has in its possession, and may have shared with third parties. Personal information is defined as information that identifies, relates to, describes, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular consumer.
