What Happened?

Mission Neighborhood Health Center (“MNHC”) recently learned that it was affected by a data security incident involving the systems of a third-party vendor. On December 12, 2025, MNHC was notified by OCHIN, its business associate that supports MNHC’s Epic electronic health record system, of a data security incident involving TriZetto, a healthcare eligibility and claims clearinghouse and subcontractor of OCHIN.

According to information provided to MNHC, the incident involved unauthorized access to certain TriZetto systems that occurred in November 2024. While MNHC’s own systems were not directly accessed, the incident may have involved protected health information belonging to MNHC patients. TriZetto reportedly identified and contained the incident and secured its systems on October 2, 2025. OCHIN became aware of the incident on December 9, 2025, and subsequently notified MNHC on December 12, 2025, at which time MNHC received a list of potentially affected individuals.

Upon receiving notice, MNHC promptly began its investigation, mitigation, and notification efforts in accordance with state and federal law. On December 19, 2025, MNHC filed an official notice of the data breach with the U.S. Department of Health and Human Services’ Office for Civil Rights, as required under federal reporting guidelines for breaches involving protected health information.

Although details about the incident remain limited, public reporting indicates that the breach may have impacted the protected health information of more than 3,700 individuals.

Notification letters have begun being sent to affected individuals, providing details about the incident and the types of information that may have been impacted.
