On October 2, 2025, TriZetto Provider Solutions (TPS) became aware of suspicious activity involving a web portal used by certain healthcare provider customers to access TPS systems. TPS determined that an unauthorized individual had been accessing certain records as early as November 2024, and that this activity continued until the incident was discovered. Upon discovery, TPS promptly launched an investigation, took steps to mitigate the issue, engaged external cybersecurity experts, and notified law enforcement. Following a comprehensive review of the affected data and the individuals involved, TPS notified Gardner Health Services of the incident on December 9, 2025.
According to TPS, the information that may have been involved includes individuals’ names, addresses, dates of birth, Social Security numbers, health insurance member numbers (which, for some individuals, may include Medicare beneficiary identifiers), provider names, health insurer names, primary insured information, and other demographic, health, and health insurance information. The notice states that no payment card information, bank account details, or other financial information were affected. At this time, TPS has indicated that it is not aware of any identity theft or fraud resulting from the incident.
